It's the day after the events of the first film and Sally Burns and her friends are preparing for a Halloween party. As the evening of fun kicks off, unbeknownst to them, The Plague Doctor and The Creature are preparing to join them.

When her parents decide to separate, sixteen year old Anna Brooks, a narcoleptic, is sent to spend the summer in a small coastal town with her aunt and uncle, Jennifer and David, who hold some very antiquated values. Suffering traumatic hallucinations, Anna also witnesses several bizarre and sinister incidents involving the townspeople, creating palpable feelings of paranoia and anxiety, leaving Anna unsure of who to turn to for help.
